（1）《进口棉花境外供货企业登记申请表》

1. Application Form for Provisional Registration of Overseas Supplier Enterprises of Imported Raw cotton;

Note1: The form need to be signed by legal representative with company chop

（2）有效的合法商业登记文件（复印件）：商业登记文件指申请企业的商业登记（复印件），并附中文说明，说明登记证书种类、证书内容、发证机关、发证日期等。

2. Business registration documents (Certificate of incorporation and IEC certificate)

Note: If you have ISO9001:2008 certificate, also provide it.

（3）稳定的经营场所或加工场所的资料（平面图）：指固定办公场所地址、标有尺寸的平面图照片

3. The plan and some photos of the permanent plant (including warehouse and processing locations) and office

Note2: The size (length and width) of the permanent plant (include warehouse and processing locations) and office must be marked on the plan. The photos must show the company’s name, the gate, the warehouse, the processing locations, the facility, and so on. The more the better.

（4）棉花检验报告（如是自检的需提供检验设备的型号及照片)

Cotton inspection report (if it is self inspection must provide inspection e model and photos)
